CAP Intakes are held in two back-to-back sessions.

Orientation – The First Step

Come to this small group session to learn about what services CAP provides to people living with HIV.  The orientation takes about 30 minutes.  Please arrive early to complete necessary paperwork. Immediately afterward, you’ll be connected to a staff member who can hear about the specifics of your situation, and learn how best to help you by doing a brief assessment.

Assessment – The Second Step

Our one-on-one appointments that follow orientation can take up to an hour and provide an individualized needs assessment for new and returning clients.  These assessments also give information and referrals for CAP programs, medical case management and many other HIV services in the Portland metro area.

If you’d like to fill out CAP’s Intake Form in advance to bring with you to your appointment, please click here.  For CAP Orientation appointments, please bring with you a photo ID, proof of income (if any), verification of your HIV status, and any insurance documentation.
