Intake – the First Step

This assessment is the initial access point for all CAP services.  Intake appointments typically take about an hour and provide an individualized needs assessment for new and returning clients.  Intakes give information and referrals for CAP programs, medical case management and many other HIV services in the Portland metro area.

If you’d like to fill out CAP’s Intake Form in advance to bring with you to your appointment, please click here.  For CAP Intake appointments, please bring with you a photo ID, proof of income (if any), verification of your HIV status and any insurance documentation.

Information and Referral

The Oregon AIDS/STD Hotline is also available to answer questions regarding HIV services and issues for curious individuals and other agencies.
