Latino Medical Case Management
What it is: CAP Latino Medical Case Management works closely with primary HIV case managers to serve Latinos living with HIV/AIDS. Our staff specializes in serving people with high needs including: language and cultural barriers, homelessness, severe medical needs, mental illness, substance abuse, and lack of adequate insurance.
What it provides: Culturally specific advocacy, benefits counseling, support accessing medical care (including transportation to medical appointments), linkage to mental health and substance abuse services, adherence counseling, home visits, housing resources and education.
Who it assists: Latinos living with HIV/AIDS who have high needs (acuity score of >29).
Where it provides services: Serves medical sites throughout Multnomah, Clackamas, Washington, Yamhill, Columbia and Clark County.
Why it is special: Latino Medical Case Management provides culturally specific, intensive case management. Services are provided where clients most need them – in the clinic, hospital or at home.
